Once you have completed this form and sent it off to us, one of our trained Sales Associates will contact you with pricing via e-mail, so please be sure that you insert your correct e-mail address.  In the e-mail response we will then provide you pricing, availability and expected ship date.  For all Internet orders we do require pre-payment in the form of VISA or MasterCard.  Please call one of our Sales Associates between the hours of 8am and 5pm ET, Monday through Friday, and pay by either your VISA or MasterCard information by phone.  We ask this so as to ensure your safety with credit card information, as e-mail is not a secure method for transmissions of such confidential items.  Although this process of pre-payment may seem inconvenient, we have found that due to the custom nature of most of our product line that this order process helps to ensure complete customer satisfaction.
